4 Leukotriene Modifiers and Mucolytics
Compare leukotriene modifiers and mucolytics by mechanism, clinical use, and key safety precautions.
Leukotriene effects and clinical role
Leukotrienes contribute to airway inflammation, bronchoconstriction, mucus production, and airway swelling. Leukotriene modifiers reduce leukotriene effects or production and are used for prevention or ongoing asthma control, not to relieve an acute asthma attack. They do not replace a patient's prescribed acute-relief plan.
How leukotriene modifiers work
Montelukast and zafirlukast block . Zileuton acts differently: it inhibits , reducing leukotriene synthesis.
These medications are not rescue bronchodilators. For allergic rhinitis, montelukast is generally reserved for patients who do not respond to or cannot tolerate alternatives.
Important precautions and interactions
Montelukast carries a boxed warning for serious neuropsychiatric events. Discuss this risk and monitor for new symptoms, including nightmares or marked mood changes. New symptoms need prompt attention; prescribing information advises discontinuing the medicine and contacting a health professional if they occur.
Zafirlukast and zileuton can cause liver injury. Zafirlukast can increase prothrombin time in a patient taking warfarin, so monitoring is important. Zileuton can raise theophylline and warfarin levels; relevant monitoring and dose adjustment may be needed.
and thick secretions
is an inhaled mucolytic. Its sulfhydryl group disrupts disulfide bonds in mucus proteins, reducing mucus thickness and viscosity.
Inhaled can cause unpredictable . Monitor the patient, and discontinue the medicine if progresses. Obtain clinical assessment; a bronchodilator may be used as directed.
in cystic fibrosis
is a recombinant DNase used with standard therapies for cystic fibrosis (CF). It breaks down extracellular DNA in CF sputum, reducing sputum viscosity and viscoelasticity and helping improve pulmonary function.
Choosing and using mucolytics
Distinguish the mucolytics by what they break down: disrupts disulfide bonds in mucus proteins, whereas cleaves extracellular DNA in CF sputum. Follow product-specific instructions and current prescribing information when using these medicines.
